About our self-request pathology tests
QML Pathology offers a limited range of self-requested pathology. In certain circumstances, a person may request to have a pathology test performed without the authorisation of a doctor or registered practitioner. There is no Medicare rebate for self-requested tests with payment required in full at the time of collection.

No. However, if results are found to be grossly abnormal you may be contacted for authorisation to have results forwarded to your Medical Practitioner.
As per The Public Health Regulation 2005 (Schedule 1), pathology laboratories are required to notify the Queensland Notifiable Conditions Register for each condition that is currently notifiable under the Queensland Public Health Act 2005 and Public Health Regulation 2005. The list of notifiable conditions is available from www.health.qld.gov.au.
Disclaimer: It is important to note that Pathology tests are usually requested by a patient’s doctor as they are medically qualified and trained to interpret and/or analyse the results after a full assessment of the clinical signs and symptoms a person may be experiencing. Some panels of testing are quite complex. There may be the possibility of misinterpretation of pathology results in the absence of advice from the appropriately trained and qualified Medical Practitioner. Patients using self-requested testing will need to consult their local Medical Practitioner if they require interpretive advice. QML Pathology does not provide any interpretive services for results of self-requested tests.

At the time of sample collection you can choose from two options:
- Post: Results will be delivered via Australia Post to the patient address indicated on the request form.
- In-person: You can choose to collect a hardcopy of the results from the Collection Centre where your sample was taken. To collect results in person you will be required to show photo identification.
Please note: Due to private and confidentiality policies patient results CANNOT be emailed.

Full payment is required at time of collection. Payment can only be made by Credit Card.

**However, there are 11 tests that fall under the Exemption Test Fee, these tests are complex and will be charged at a higher rate than $100. This charge will be in addition to any standard fee tests requested. Please see the table for the list of these tests.

There are no Medicare rebates for self-requested testing, all fees must be paid in full at time of collection.
Each episode (Collection) will incur a $20.00 collection fee.

Yes, for a limited number of tests you are able to self-request if you are aged 16 or over. QML Pathology does not perform self-requested tests on persons under 16 years without their legal parent/guardians’ authorisation.

- Select the region where you will have your test from the drop down menu.
- Choose the Patient Self Request form for the test you require. The General Self Request Form has a number of pre-selected testing options.
- Download the form to your desktop and complete the required details. You will need to enter your name, current postal address, date of birth and contact details.
- Select which test/s you require from the list on the form. Please choose carefully as QML Pathology take no responsibility for incorrect forms.
- Print the completed form and take it to a QML Pathology collection centre.
